After 38 months of cross-border collaboration, HealthChain http://healthchain-i3.eu/ has come to an end, leaving behind a portfolio of validated digital health solutions, stronger opportunities for SMEs, and a replicable model for accelerating healthcare innovation across Europe.

At ZenLab, we developed MOBILECARE+ with support from Biromatik as part of the HealthChain project, helping patients stay connected with healthcare professionals during recovery after hip surgery. MOBILECARE+ is a patient-centered mobile app designed for real-time monitoring of rehabilitation progress. By integrating data from wearable devices, the system provides healthcare professionals with actionable insights into patients’ recovery. This allows for personalised adjustments to treatment plans.
